The 60-year-old man first shot his son, and then went from house to house in the village of Velika Ivanca gunning down his neighbours and relatives, according to Serb media.
Officials said six men, six women and a two-year-old boy comprised the victims. Twelve died at the scene while another died in hospital.
The shooting is the worst peace-time massacre in the history of the modern Serb state.
"There are a total of five houses affected where neighbours and relatives lived," an official told the B92 news website The gunman, identified only as Ljubisa B, then tried to kill his wife before turning his gun on himself. Both survived although remain in a critical condition.
"Doctors are fighting to save the lives of two people, one of whom was the perpetrator of this terrible mass murder," said Milorad Veljovic, the regional police director. "The investigation has started and we are trying to ascertain the motive for this bloody crime."
The policeman described Ljubisa B as a normal family man who had no criminal record, while doctors added that the gunman had not been treated for psychological problems.
Neighbours of the man told B92 he never shown signs of violence and he had always been "perfectly normal." One said something must "have flicked in his head."
Despite the prevalence of firearms left over from the Yugoslav wars shootings are rare in Serbia so the massacre at Velika Ivanca has shocked the country.
